• - Cold Plunge

• - FloPresso Lymphatic Massage

• - I Am Rooted Wellness services are not intended to diagnose, treat, cure, or prevent any medical condition. These services are designed to support overall wellness and prevention.

• - It is my responsibility to consult a licensed medical professional or my primary care provider before participating in any services if I have any medical conditions, concerns, or am taking medications that may affect my safety.

• - The practitioners at I Am Rooted Wellness cannot provide medical diagnoses or prescribe treatments.Acknowledgment of Services and Risks

  I understand that:

• - Services such as red light therapy, infrared sauna, grounding mats, and ionic foot baths are holistic and wellness-based. Results are not guaranteed and may vary from person to person.

• - Potential risks may include, but are not limited to: mild discomfort, skin irritation, or detox symptoms during or after services.

• - Infrared sauna use may pose risks for individuals with certain conditions (e.g., heart disease, high blood pressure, pregnancy).

• - I confirm that I have disclosed any relevant medical conditions, medications, or sensitivities that may impact my ability to safely use these services.

  Client Responsibility

• - Seek medical clearance from my physician or healthcare provider prior to participating in any services, particularly if I have pre-existing medical conditions, am pregnant, or am taking medications.

• - Inform I Am Rooted Wellness staff of any new medical conditions or symptoms that arise during my participation in services.

• - Recognize that holistic wellness services are complementary to, not a replacement for, professional medical care.

  Assumption of Risk and Waiver of Liability

  I voluntarily assume all risks associated with participating in services at I Am Rooted Wellness. I hereby waive, release, and hold harmless I Am Rooted Wellness, its owners, staff, and affiliates from any and all liability for injuries, damages, or claims resulting from services provided.

  Consent to Services

  I consent to receive the services offered at I Am Rooted Wellness and understand that I may withdraw consent at any time.
